JOB SUMMARY:
Ozanam Manor (I and II) Interim Housing Communities are transitional housing programs for 160 single adults experiencing homelessness who are 50+ or disabled, including Veterans. Nursing care is necessary for homeless Veterans who have some type of unstable or uncontrolled medical issue that requires a level of oversight. The Ozanam Nurse's primary responsibility is to provide designated Veterans with high quality medical and mental health recuperative care in accordance with the Veteran's Administration Medical Recuperative Care Contract (approximately 20 hours per week). For the remainder of the position, the Ozanam Nurse is responsible for the overall healthcare oversite for the program, primarily through consultation to staff regarding Resident medical concerns, but with more direct oversight for residents with high medical needs.

SPECIFIC DUTIES:
Medical Recuperative Care Contract Resident Veterans
(No less than 20 hours per week for up to 5 Residents)
The Ozanam Nurse is responsible for following the plan of care developed by VA referring healthcare providers. This includes medication management, monitoring vital signs and coordination of care with VA medical staff. The services to be provided shall include but are not limited to:
