Care Management Coordinator


Location: Harrisburg, PA | Onsite with local community travel
Schedule: Monday-Friday, 8:00 a.m.-4:30 p.m.
Pay: $20.00 per hour
Employment Type: Temp-to-permanent | Initial six-month assignment
Mileage Reimbursement: Federal mileage rate for required community travel


About the Role


Abel Personnel is hiring a Care Management Coordinator for a confidential Dauphin County-based aging and community services organization. This onsite role supports a community-based care management and protective services program through client outreach, care plan coordination, accurate case documentation, and regular local field visits.

This opportunity is a strong fit for someone who communicates calmly, gathers information thoughtfully, and can adjust questions in real time. You will balance office-based coordination with community travel while helping individuals access appropriate services and support.

What You Will Do


  • Accept and screen incoming calls, referrals, intakes, and walk-in inquiries.
  • Conduct telephone and in-person interviews with clients and other individuals involved in active cases.
  • Review active care plans, identify changing needs, and help coordinate appropriate services and community resources.
  • Travel locally a few times each week to meet with clients and complete in-person care plan reviews.
  • Place outbound calls related to care coordination, follow-up, and case investigations.
  • Ask appropriate follow-up questions and adjust conversations based on client or collateral responses.
  • Document case activity, interviews, and client interactions accurately in the computerized client information system.
  • Maintain professional working relationships with community agencies, service providers, families, and support networks.
  • Follow established requirements for confidentiality, documentation, client care, and professional conduct.


Required Qualifications


  • Bachelor’s degree in Social Work, Sociology, Psychology, Gerontology, Human Services, or a related social science; or an equivalent combination of education and relevant experience.
  • Strong communication and interviewing skills in both telephone and face-to-face settings.
  • Ability to process information quickly, solve problems, and adapt questions based on the conversation.
  • Ability to work independently while managing multiple cases, tasks, and priorities.
  • Basic computer proficiency, including data entry and electronic case documentation.
  • Ability to work respectfully and professionally with individuals experiencing physical, emotional, or social challenges.
  • Valid driver’s license, reliable transportation, and a clean driving record that meets assignment requirements.
  • Ability and willingness to travel locally for required client visits and community-based work.


Preferred Experience


  • Experience in care management, case management, protective services, aging services, social services, community outreach, or a related human services setting.


Work Environment and Expectations


  • Onsite office environment in Harrisburg with free parking.
  • Monday-Friday schedule from 8:00 a.m.-4:30 p.m.
  • Combination of desk work, telephone outreach, electronic documentation, and local field visits.
  • Community travel a few times per week, with required mileage reimbursed at the federal mileage rate.
  • Frequent interaction with clients who may be distressed or navigating complex circumstances.
  • Requires professionalism, sound judgment, flexibility, empathy, and careful handling of confidential information.
