Siouxland Community Health Center has an opening for: Prenatal Behavioral Health Case Manager 1- Full-time M-Fri 8am-5pm Successful candidate must have solid experience & advanced knowledge of: S.W. or appropriate Bachelors level degree. Able to educate on diabetes management. Certified car Seat Safety Specialist. Current BLS and Mandatory Reporter certifications Minimum of two years experience in case management or related experience. Understanding of socio-economic, culture, and other barriers to medical care and ability to diminish. Knowledge of community resources and willingness to develop relationships with external agencies. Successful candidate must be able to perform primary functions of position: Evaluate Behavioral health surveys patients complete during their OB visits and identify stressors for that patient.
