PRENATAL BEHAVIORAL HEALTH WORKER

Job Description

Siouxland Community Health Center has an opening for:
Prenatal Behavioral Health Case Manager
1- Full-time M-Fri 8am-5pm

Successful candidate must have solid experience & advanced knowledge of:
  • S.W. or appropriate Bachelors level degree.
  • Able to educate on diabetes management.
  • Certified car Seat Safety Specialist.
  • Current BLS and Mandatory Reporter certifications
  • Minimum of two years experience in case management or related experience.
  • Understanding of socio-economic, culture, and other barriers to medical care and ability to diminish.
  • Knowledge of community resources and willingness to develop relationships with external agencies.
Successful candidate must be able to perform primary functions of position:
  • Evaluate Behavioral health surveys patients complete during their OB visits and identify stressors for that patient.
  • Identify socio-economic risk factors.
  • Identify and intimate partner risks.
  • Screen for history of drug abuse currently or in the past and offer assistance for mothers to seek help.
  • Make referrals to HOPES program for qualifying patients.
  • Independently email foundation about patients' needs outside of the nursing staff.
  • Work alongside nursing staff and triage patient visits and not let nursing dictate her work.
  • Assist those with food insecurities with Food Bank of Siouxland Website finder for pantries close to home.
  • Reach out to Mary's Choice, Salvation Army, and the Community Action Agency for formula assistance in addition to WIC for those families struggling with finances.
  • Assist families with needed material and education regarding WIC and ensure they are able to make an appt with WIC post delivery.
  • Ensure PCP is set up for baby for pediatric care .
  • Identify patients who have a need for car seat and crib and assist with getting such items.
  • See patients who have elevated phq, GAD 7, or Edinburgh assessment score before departure from clinic.
  • Independently know how to get baby scheduled to see PCP if parents have not had the child seen in the clinic.
  • Assist providers in patient care as directed.
  • These functions are not all inclusive
