Join our team as a Health Education Class Facilitator at Coastal Family Health Center in Astoria, OR.  This position provides and facilitates Chronic Disease Self-Management Program (CDSMP) classes for community members in assigned geographic areas. This is a part-time position approximately 4 hours/week, and requires the ability to work evenings and occasional weekends.
