Introduction & Job Duties This opportunity is for County Caseworkers to provide casemanagement to individuals meeting criteria for developmental delays, mental health, and/or with intellectual disabilities. Responsible for monitoring, linking, and evaluating services. Link to other services and make referrals as needed. Provide regular direct individual contact and provide face-to-face casemanagement services. Meet productivity standards as set for the program. Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family. Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy.
