Job Description Job Description Responsibilities: Provide advocacy,linkage,and referral services on behalf of chronically mentally ill.. Provide intake, screening, referral and case management services. Assist in maintaining an active liaison with community agencies serving the “high risk” population with modified PACT program. Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS rules and regulations. Attend and regularly present cases in the treatment team meetings. To read and be knowledgeable of all current CREOKS policies and procedures. Maintain productivity averages of direct service to clientele as set by CREOKS for full time employment. Coordinate and manage indigent medication/patient assistance program. Turn in billing and associated documentation as mandated by CREOKS policy.
